Abstract Aim Of The Work: To study the oesophageal mucosal levels of IL 8 in patients with reflux oesophagitis (RE). Summary, Conclusion GERD is a condition that develops when the reflux of stomach contents causes a troublesome symptoms and /or complications. The esophagus carries food and liquid into the stomach. At the lower end of the esophagus where it enters the stomach, there is a strong muscular ring called the lower esophageal sphincter (LES). The LES should remain tightly closed, except to allow food and liquid to pass into the stomach. Chemokines are a family of small cytokines, or proteins secreted by cells. IL-8 exhibits a potent chemotactic activity for neutrophils and plays a critical role in the induction of acute and chronic inflammatory reactions. The aim of this work is to study the oesophageal mucosal levels of IL-8 in patients with reflux oesophagitis. This study was done on 50 subjects (40 patients with GERD symptoms and 10 healthy subjects) of both sexes from those attending the tropical medicine out patient clinic of El-minia university hospital. The subjects classified into 3 groups: group A include 20 patients with reflux oesophagitis, group B include 20 symptomatic patients with non erosive GERD and group C include 10 healthy subjects (control group). All subjects included in the study were subjected to clinical history, examination, abdominal ultrasonography and upper endoscopic examination. The biopsy specimens are used for determination of levels of mucosal levels of IL-8 by immunohistochemistry and another for histopathological examination. Regarding the endoscopic findings in GERD patients:. 45% of patients were of grade A, whereas grade B was found in 25% of patients. 20% were presented with grade C. Grade D was found in 10%. Hiatus hernia in 30% of patients. Histopathological findings showed the following: 2 patients showed no abnormality detected. 16 patients showed grade 1 oesophagitis. 4 patients showed grade 2 oesophagitis while 12 patients showed grade 3 oesophagitis. There are 6 patients with grade 4 oesophagitis. We found a significant increase in IL-8 expression in patients with GERD than NERD and controls. Also, there is a significant increase in IL-8 levels in smokers than non smoking patients. We may conclude that there is a relationship between IL-8 and reflux disease. Therefore, it is recommended that more studies are needed for further understanding of the other mechanisms and methods for detection of IL-8 in patients with reflux disease &other inflammatory conditions. |
