Abstract

It is believed that THA has made a revolution in the way of treatment of hip arthritis. With an increasing number of younger patients undergoing THA, the incidence of revision hip surgery is on the rise. Revision THA is a technically difficult procedure and associated with higher complication rates.
The aim of this study was to evaluate the results of long stem femoral component in revision THA.
This study included 20 patients 15 men (75%) and five women (25%) with a mean age of 61.8 years ranging from 50 to 75 years. The right side was affected in 12 patients (60%) and the left side was affected in eight patients (40%). Nine patients (45%) were retired employees, six (30%) were employees and five (25%) were housewives. The mean of the patients’ BMI was 25.9, ranging from 20.6 to 31.2. Two patients (10%) were diabetic and two patients (10%) were hypertensive. Lateral approach was used in 18 patients (90%) while posterior approach was used in two patients (10%).
Indication for revision was aseptic loosening in 13 patients (65%), septic loosening in four patients (20%) and recurrent dislocation in three patients (15%).
The mean evaluation period was 35.75 months (range from 12 to 56 months). The patients were evaluated both clinically using Harris Hip Score and radiologically.
The mean preoperative HHS was 46.2 (range from 24 to 60) and the mean HHS in the last follow up was 78.35 (range from 44 to 95). Excellent results (HHS >90) were obtained in 6 patients out of 20 representing 30% of the studied group, and good results (HHS 80-90) were reported in 5 patients representing 25%, fair results (HHS 70-79) encountered in 5 patients representing 25%, while poor results (HHS <70) were reported in 4 patients representing 20%.
