Abstract

This study was carried out on 483 blunt poly traumatized patients admitted to the emergency department at period from 1/12/2012 to 31/5/2013 to study the patterns of poly trauma, mode of trauma, integration of management and fate of those patients.
For all patients, general examination was carried out including vital signs and RTS score and systemic examination. U/S abdomen and CT was carried out for the poly traumatized patients to take decision whether surgical or conservative management needed.
The result of this study revealed that:
• Male to female ratio was 2:1.
• Young patients are more susceptible to poly trauma (mean 29.91 ± 16.57).
• RTA representing 79.7% of our patients as a mode of trauma.
• The most common type of injury was head injury representing 98.8%.
• Surgical management was carried out for 82.4% of patients while conservative management was executed in 17.6% of patients.
• Abdominal surgeries were carried out in 9.5% of the patients.
• Moderate abdominal collection was the most common finding before laparotomy.
• All patients with minimal abdominal collection were treated conservatively.
• Conservative treatment in cases of splenic laceration was done for 68 patients while surgical for 12 patients.
• Bowel injury was found in 19 patients, 13 were treated by direct intestinal repair while 6 with resection and anastmosis.
• The survival rate in our study was 90.9% (439 patients), while 9.1% died (44 patients).
• The main cause of death was intracranial heamorrhage representing 77.2% of the deaths.
• Death rate was significantly high in ages below 25 years.
• Falling from height as a mode of trauma was a significant cause of death in our patients.
• from the died patients 42 (95.5%) were treated conservatively, while from the cured patients 329 (90.2%) were treated surgically.
