Abstract

This study was conducted for evaluation of management of ureteropelvic junction obstruction by different surgical approaches. In our study, we have four groups, first is open group, third is conventional laparoscopic group, both were done at Menofiya University hospitals. However, the second group (Robot-assisted pyeloplasty) and fourth group (conventional laparoscopic pyeloplasty) both were done at Cleveland Clinic, Ohio, USA. In the prospective non randomized part of this study, we compared first, second and fourth group with each other. In the retrospective part of the study, we compared laparoscopic pyeloplasty in Menofyia university hospitals, Egypt (group 3) and laparoscopic pyeloplasty in Cleveland clinic (group 4). We investigated the influence of all preoperative, intraoperative and postoperative variables on short term success and early recurrence. Success is defined as symptom resolution and improved radiological or renographic parameters during the follow up period. In our study we considered three parameters for success, clinical, radiological and functional. Functional parameter measures both ipsilateral GFR % and T ½ minutes. Subjective and objective evaluation of the outcome was carried out after 3 months and one year then every year for 3 years. For first set of comparison between first and second groups, age and preoperative split function, morphine equivalent dose and operative time was statistically significant, however, intar and post-operative complication were comparable in both groups For first and fourth groups, age and preoperative T1/2, morphine equivalent /dose and operative time was statistically significant, however, intar and postoperative complication were comparable in both groups. Also, post-operative T1/2 time and post-operative GFR % at 3 months follow up was statistically significant between both groups.The last set of comparison in the prospective study was between second and fourth groups, only operative time was statistically significant in favor of Robot-assisted pyeloplasty, however, all pre-operative, intraoperative, and post-operative variables were comparable without statistical difference. In the retrospective study, we collected the data for third and fourth group, only age and estimated blood loss were statistically significant in both groups. Finally, success rate in four groups were comparable to published series.
