Abstract It was found that adding certain drugs to the local anaesthetics in peribulbar block increases its intensity and duration of action, so we have done this research to evaluate the effect of adding fentanyl to the local anaesthetics in PBA for vitrectomy. Our study was performed in El-Minia University Hospital in the period from January 2013 to October 2013 on forty patients undergoing vitrectomy surgery of both sexes, their ages ranging between 40 to 80 years old and ASA from I to III. Infomed consent was taken from these patients to undergo this operation under local anaesthesia (PBA). Patients were randomly and blindely classified into two equal groups: Group A: They received volume of 9.5 ml (2:1 mixture of bupivicaine 0.5% (6 cc) lidocaine 2% (1.5cc) + 45 IU hyalurinidase (in 1.5cc lidocaine) + 0.5cc saline). Group B: They received volume of 9.5 ml (2:1 mixture of bupivicaine 0.5% (6 cc) lidocaine 2% (1.5cc) + 45 IU hyalurinidase (in 1.5cc lidocaine) + 25 ug (0.5) cc fentanyl). The block was done by the transconjunctival, double injection approach of PBA . Patient’s demographic data (sex, age and weight), duration of surgery ,haemodynamics and oxygen saturation, motor block onset , globe and lid akinesia scores (at 2,4,6,8 and 10 minutes after injection), the block, quality of anaesthesia , patient’s satisfaction scores and post operative VAPS and analgesic request in each group were recorded. |
