Abstract

Most hand fractures can be treated by non operative methods with good outcome only less than 5℅ of these cases which are unstable fractures need open reduction and internal fixation where conservative traetment are usually unsatisfactory.
The purpose of this work is to evaluate the results of using the miniature plates and screws in management of closed fractures of metacarpal and phalangeal bones concerning, The stability, early movement of hand , incidence of complications and healing.
The study was carried out from April.2012 to July 2013 in the department of Orthopedic surgery and Traumatology in Minia University Hospital. It includes 10 cases of recent metacarpal and/or phalangeal fractures from 2nd to 5th ray where five cases had proximal phalnex fracture, three cases had metacarpal bone fracture and two cases had middle phalnex fracture , all cases were treated by open reduction and internal fixation using miniature screws and plates.
Radiographic evidence of union was observed at 3 months, one delayed-union was observed in one case (10%),while complete union was achieved in nine cases (90%) .
Hand function was performed at the final follow up according to Active ROMs and the results are satisfactory, with over 80% of patients gaining good results.
According to the final results of this study, we consider this procedure as a good and reliable method
In conclusion, our series of metacarpal and phalangeal fractures treated with miniature plates and screws, very favourable outcomes should be expected, and this type of fixation could be a good indication for this subgroup of fractures.
The stable bony construct achievable by using miniature plates and screws is the key to good functional results. Active mobilisation can be started immediately after surgery and tendon gliding can be preserved.
