Abstract The present study had been performed at the pediatric department , out patient clinic , intermediate care unit , and neonatal intensive care unit (NICU) of Sohag university hospital , Upper Egypt through the period from 1st of January 2012 to the end of January 2013. It included 40 patients at age from birth to 12 years , of them 18 (45% ) were males ,while 22 (55%) were females with female predominance ; male to female ratio was [1: 1.2] . Their ages at presentation ranged from 0 days to 12 years Most of studied cases (65%) presented during infancy, the least common risk factors among studied cases was positive consanguinity in (20%) . The majority of cases had complete AVCD , followed by partial AVCD Of the 40 patients ,(10%) were asymptomatic and discovered accidently during routine medical examination . The most frequent symptoms were reccurent chest infections in (50) of patients , while the less frequent was sleeping disorder , , RD was the commonest in (875%) of cases , followed by systolic murmer (65%) , and the less common was sign of PHN (12.5%) of cases. As for investigations , ECHO was the one of choice and revealed that size of the defect , ranged from ( 6 - 10 mm) were the commonest . Chest x- ray demonstrated that most of cases had cardiomegaly , and ↑ broncho- vascular markings i.e. (lung plethora ) , while ECG revealed that the vast majority of cases had tachycardia with no arrysthmia Extra-cardiac anomalies (ECAs) , In our study, were encountered among (37.5 %) of cases had down syndrome was the commonest, whereas the most common complications recorded were, recurrent chest infections in ( 50%) of cases , ,and the less frequent were arrhythmia Conclusion: The great majority of infants with AVCD are most often symptomatic. Frequently, the condition is detected by a murmur on routine physical examination when they had RD.. In younger children, subtle failure to thrive may be present, in older children, varying degrees of exercise intolerance may be noted . |
