Abstract propller flaps based on perforator vessels are raising interest in reconstructive surgery of the limbs. These flaps allow efficient coverage of large wounds without the need to sacrifice a major vascular axis. The operative technique does not require microvascular anastomosis and allows reconstruction of soft tissue defects using nearby similar tissues. The aim of this study was to evaluate the clinical results of propller perforator flaps in the treatment of various lower leg defects. In the current study34 cases of various leg defects were covered by propller perforator based flaps. Defects were due to Trauma in 27 cases, gunshot injuries in 2cases, electric burn in 1 case and 4 case of a chronic non healing ulcer. Postoperatively, 20 cases healed completely. One case showed total flap necrosis(diabetic female patient), 2 cases showed partial loss of the half of the flap, and8 cases showed partial loss of the dermis and were healed. by secondary intention and 1 case had DVT and was managed under medical supervision. In most cases, the aesthetic result was optimal and patients were fully satisfied. The use of propeller flaps to cover defects in the lower leg showed satisfactory results as regards flap viability and providing a suitable cover of various defects. The use of hand held Doppler proved satisfactory in pre operative delineation of the perforators but colour duplexU/S is more accurate and less false positive or false negative . The use of propeller flaps proved to be effective as regards viability of the flap and decreasing the donor site defect. |
