Abstract

High tibial osteotomy surgery is an operation where a bone is cut and Repositioned. In a high tibial osteotomy the bone of the upper tibia is cut and reangulated to realign the leg. This reduces the pain that is experienced in the knee by taking pressure off the damaged joint compartment. The aim of this study is to explore and developing postoperative nursing care standards for patients undergoing high tibial osteotomy surgery. To accomplish this aim; data were collected from all baccalaureate nurses orthopedic surgery department and all diploma nurses who working in orthopedic surgery department . The following tools were utilized for data collection. Tool I: Nurse’s questionnaire sheet: (Annex I) It was developed by the researcher based on current national and international literatures it includes two parts Part I: Socio-demographic data: This part comprised nurse’s name, age, sex, marital status, level of education, duration of experience. Part 2: Assess nurse’s knowledge regarding high tibial osteotomy: Such as definition of high tibial osteotomy, causes, management, complications, and nursing care and instruction for patients. Part 3: Observation nursing practice checklist sheet: This sheet was developed by the researcher to assess nurse’s performance it include preoperative care immediate and postoperative care and instruction for patients before discharge about surgical site care, exercise, maintain rest and comfort, any developed complications and routine follow up. Summery 69 Immediate post-operative nurses practice competencies This part includes assessment patient’s respiratory function, patient’s circulatory function, thermoregulatory function, neurological function, gastrointestinal function, elimination function and level of comfort. Routine post-operative nurses practice competencies. This part includes the following: relieving pain, wound care, Transfer to wheelchair, using of knee Brace, using of knee immobilizer, using of cold application, exercise, using of crutches, avoid postoperative complication. Before discharge nurses practice competencies. This part includes health education before discharge regarding Showering, Dressing, Elevation, Ice, Crutches, Medication, Home modification, Diet, exercise, recognition of complications and follow up care. Tool II: Construction of developed postoperative nursing care standards Postoperative care standards will be developed according to the needed nurse’s practice that can help nurses in provision of a safe and effective care for patients who had high tibial osteotomy. Tool III: Post-operative Opinionnaire sheet for nurses (POS) (Annex II) It was developed by the researcher based on current national and international literature in order to elicit opinions of two groups as regard to the basic nursing competencies for post-operative high tibial osteotomy patients. This tool covered 7 major broad competencies which were further subdivided into sub competencies 1.Ensure that the orthopedic surgery department environment are ready to receive the high tibial osteotomy patient Summery 70 2.Ensure that the orthopedic surgery department environment are safe to receive high tibial osteotomy patients. 3.Ensure that continuous monitoring and recording are followed for post operative patients. 4.Ensure that all infection control measures are properly followed in all procedures . 5. Ensure that all health team are properly attired for patient care. 6. Ensure that safety for each patient during transportation from operating room to unit. 7. Ensure that all staff (health team) follow ethics and patients right in the orthopedic surgery department. Main findings of the present study were: As regard to Sociodemographic data, the more than half of nurses 56.7 their age ranged from 20- 40 years. 90.0 of them were married. 90.0 of them had more than 5 years of experience. 70 of them had diploma of nursing. As regard to previously of attended training programs, it was found that 96.7 were not attending training program about high tibial osteotomy. As regard to other training course. As regarding nurses; knowledge about standards of post operative interventions for high tibial osteotomy patients, more than half of nurses had good knowledge regarding standards of post operative interventions for high tibial osteotomy patients. As regarding to relationship between knowledge about high tibial osteotomy and Sociodemographic characteristics there was significant between level of education and knowledge with P.value (0.083) and significant between years of experience and knowledge with P.value (0.007). A regard to nurses’ practice regarding post operative interventions for high tibial osteotomy patients, nurses showed inadequacy in their practice about standards of post operative interventions for high tibial osteotomy patients. 2.Ensure that the orthopedic surgery department environment are safe to receive high tibial osteotomy patients. 3.Ensure that continuous monitoring and recording are followed for post operative patients. 4.Ensure that all infection control measures are properly followed in all procedures. 5.
