Abstract

Background: The diagnosis of carpal tunnel syndrome is mainly clinical,but nerve conduction studies are considered to be the gold standard for carpal tunnel syndrome diagnosis because of their high specificity and sensitivity percentages, one of the methods that were considered is nerve conduction study after provocative tests.
Patients and methods: Hundred hands of outpatient clinic patients complaining from tingling sensation along the median nerve distribution in hand, and scores 3 or more on the Levine’s scale questionnaire and a Control group of fifty hands of healthy volunteer individuals with no clinical findings concerning carpal tunnel syndrome symptoms and signs.
Results: It was found that significant changes occurred in motor amplitude, sensory and motor distal latencies, in patients compared to control group, and when divided into two groups according to positive and normal electrodiagnostic findings, there was no value added after the phalen’s test with the group of normal electrodiagnostic findings.
Conclusion: According to this study, provocation technique, using phalen test can help in better electrodiagnosis of carpal tunnel syndrome in patients with early electrophysiological changes; however it was not conclusive in patients clinically diagnosed as carpal tunnel syndrome with normal electrophysiological findings.
