Abstract

Vitamin B12 deficiency is associated with neuropathy, the progress of the neuropathy is only arrested and not reversed with initiation of vitamin B12 therapy. In the nervous system, vitamin B12 deficiency causes demyelination followed by axonal degeneration and neuronal death-not only in peripheral nerves but also in the posterior and lateral columns of the spinal cord and the cerebrum.
The aim of this work was to Study the relation between Vitamin B12 Deficiency and long term Metformin treatment in type 2 diabetic patients.
The study was conducted on 80 patients chosen from the outpatient clinic of Diabetes & Endocrinology at the Internal Medicine Hospital in Ain Shams Universityin the period from( November 2015 to December 2016)They were divided into 3 groups; group A: 20 type 2 diabetic Patients on metformin treatment for less than 2years, group B: 20 type 2 diabetic Patients on antidiabetic treatment not including for less than 2years, group C: 20 type 2 diabetic Patients onmetformin treatment for more than 10 years &groupD: 20 type 2 diabetic Patients on antidiabetictreatment not including for more than 10 years.
All 80 were subjected to full medical history, thorough clinical examination including (height & BMI), Michigan Neuropathy Screening Instrumentwas used to assess peripheral neuropathy&Laboratory investigation: Fasting and 2- hour postprandial blood glucose level, Glycated-Hemoglobin present (HbA1c)%, vitamin B12 level, ALT, AST, Serum Creatinine and BUN.
