Abstract

Multiple sclerosis is an autoimmune demyelinating disease of the central nervous system. The disorder is most commonly diagnosed between ages 20 and 40 years, but can be seen at any age.
The present study was done in Gamal Abd El-Naser insurance hospital, included 175 Egyptian MS patients diagnosed according to Revised McDonald criteria 2010, from March 2016 to March 2017.
The estimated prevalence of MS among all insured population during the investigation period was about 4.03 cases per 100.000 populations.
There were 37 new cases presented to the hospital during the period from March 2016 to March 2017.with an estimated incidence 0.95 per 100.000 populations /year.
Regarding the percentage of multiple sclerosis in relation to other neurological diseases; MS represented about 6.2 % of all cases presented to Gamal Abd El-Naser insurance hospital neurology department and out-patient neurology clinic.
The mean age of patients was 32.94 ± 8.70 years. Most of patients 119(75.8%) their ages ranged between 21 – 40 years.
55 males (35.0%) and 102 females (65.0%). Female to male ratio was 1.9:1.
Only two patients (14.3 %) had a family history of MS, 12 patients (85.7%) had a family history of rheumatoid arthritis.
There were 147 patients (93.6%) of Relapsing Remitting type (RR), 10 patients (6.4%) progressed from Relapsing Remitting type to Secondary Progressive type (SP) after 9_20 years from onset of the disease.
Regarding the first clinical presentation The most common presentation was Visual symptoms in 61 patients out of 157(38.9%), followed by sensory in 49 patients (31.2 %). 48 patients (30.6%) had motor symptoms.47 patients (30%) had sphincteric disturbance.12 patients (7.6%) presented by cerebellar symptoms.10 patients (6.4%) presented by brain stem symptoms.
There was no significant difference between male and female as regard the 1st clinical presentation, male patients had higher frequency of visual symptoms being seen in 45.5% compared to 35.3% in female patients, while female patients were more likely to present with sensory symptoms 33.3% compared to male patients (27.3%).
Highest month of first presentation was June (16.6%) and April (14.6%).
Regarding disease duration It varied from 6 months of the newly diagnosed cases to years, up to 25 years with Mean of 6.50 ± 4.26.
There was a highly statistically significant positive correlation between disease duration and EDSS.
