Abstract Background : Recurrent pregnancy loss (RPL) is one of the most frustrating and difficult areas in reproductive medicine because the etiology is often unknown. The most recent, recurrent pregnancy loss (RPL) is termed as habitual abortion and is defined as two or more consecutive miscarriages before 20 weeks gestation. Ultrasonography using power Doppler angiography accurately measures vascular parameters, including: uterine artery pulsatility index (UAPI), vascularization index (VI), blood flow index (FI), and vascularization flow index (VFI). These parameters reflect endometrial and subendometrial blood supply. Aim of this study: The aim of this study was to evaluate uterine artery blood flow and sub endometrial blood flow in women with recurrent pregnancy loss. Method: This study was a case control study carried out in the obstetrics and gynecology department of Mansoura University Hospital. The study included 60 women divided into: group 1: 40 women with recurrent pregnancy loss (RPL).group 2: 20 women as a control. All examinations were performed with the scanner with 7.5 MHZ endocavitary transducer with pulsed color Doppler and 3D facility. Examinations was performed in the second phase of the menstrual cycle between days 18–23. Results . There was a statistically significant difference regarding mean uterine artery Pulsatility index between the two groups which reflected increased resistance to blood flow in the uterine artery in RPL group. Regarding the vascular flow index (VFI), there was statistically significant difference between the two groups with higher values in control group. Conclusion: Uterine artery and subendometrial blood flow assessment seems very helpful in evaluation of patients with recurrent pregnancy loss. We hope that, improvement of uterine and subendometrial blood flow is a measure to decrease rate of recurrent pregnancy loss. Recommendations: We suggest apply uterine artery and subendometrial blood flow Doppler study as a line for assessment of patient with recurrent pregnancy loss.
