Abstract medication errors are one of the most common types of errors in healthcare systems that can cause serious harm to patients, even leading to death. The aim of this study was to identify the factors leading to medication errors as perceived by nurses and doctors at port-said general hospitals. Subjects and Method: A descriptive research design was used for the current study. The study was carried on 187 nurses and 74 doctor working at inpatient units of Port Said general hospital and El zohour central hospital. The data was collected by using questionnaire for nurses and questionnaire for physicians. Results: 90.4% of nurses reported that work environment affects negatively on medication administration, while 68.9% of doctors reported that work environment and work colleagues affects negatively on medication administration. Conclusion: Nurses and physicians exposed to multiple factors causing medication errors as work environment, colleagues, health care professional, drugs and patients themselves. . Recommendations: it is recommended providing educational programs to improve and update nurses’ and physicians’ knowledge of medication safety and medication errors, Clarity of doctors hand writing and consulting the pharmacist before prescribing drugsto avoid medication errors. |
