Abstract

• Introduction: HCV infection is considered one of the main causes of chronic liver diseases and HCC around the world. DAAs provide a great chance for HCV treatment. IL-28B has a role in antiviral immune response. IL-28B SNP can predict the response to DAAs although its role in HCC remains controversial. • Aim of the work: The aim of our study was to study the role of IL-28B SNP in prediction of HCC in patients with HCV related cirrhosis after DAAs. • Subjects and methods: The study was case control and was conducted at Specialized Medical Hospital, Mansoura University. The study included 50 HCV cirrhotic patients post DAAs induced SVR and 50 HCV cirrhotic patients who developed HCC post DAAs induced SVR in addition to 100 normal individuals as control. All selected patients were subjected to history taking and physical examination. Blood samples were collected for CBC, liver functions, AFP and IL-28B gene polymorphism PCR. Abdominal US was underwent for patients and triphasic CT for HCC cases. Results : A significant effect of HCC on deterioration of liver functions either clinically or laboratory was remarkable and high levels of AFP were detected in HCC cases. There was variation in treatment modalities of HCC according to site, size, number of focal lesions, Child classification and patency of portal vein. There was an insignificant difference between HCC group and cirrhotic group in IL-28B SNP as regards genotypes (p value=0.26) and insignificant difference as regards alleles (p value =0.77).There was a significant association in SNP of IL-28B between healthy individuals and cirrhotic group. C C and C T genotypes were more prevalent in healthy group while T T more common in cirrhotic group (p value =0.004). C allele appeared to be protective against cirrhosis while T allele was more prevalent in cirrhotic patients than normal group (p value =0.02) • Conclusion: Although IL-28B SNP had role in HCV related cirrhosis progression, it didn´t predict the probability for HCC development following DAAs.
