Abstract

Pre-eclampsia is a pregnancy-specific syndrome observed after the 20th week of pregnancy with systolic blood pressure of ≥ 140 mm Hg or diastolic blood pressure of ≥ 90 mmHg recorded twice 6 hours apart or a single measurement of ≥ 160/110 mmHg, accompanied by significant proteinuria. This was a prospective cohort study will be conducted on 50 women with severe preeclampsia and eclampsia; to evaluate maternal morbidity and mortality among patients with severe preeclampsia and eclampsia in the period from 28wk of pregnancy until labor at Department of Obstetric and gynacology at Mansoura University Hospital. All pregnant women enter Obstetric Department at Mansoura University Hospital with severe preeclampsia and eclampsia in one year from January2018 to December 2018 in pregnant women from 28week to labor with singleton pregnancy. Full history was taken from each patient as age, gravidity, parity, date of last menstrual period, present history, past history, obstetric history & family history. Also, general and local examination was done. Laboratory investigations as CBC, Coagulation profile, renal function tests, liver function tests and urine analysis for protein by (dipstick test). All women included in the study will be with sever preeclampsia (BP160/110 or more) and eclampsia to record outcome as mortality rate and morbidity as Cerebral hemorrhage, cerebral infarction which diagnosed by CT and MRI, liver capsular rapture, renal failure, HEELP syndrome, abruptio placentae, disseminated intravascular coagulopathy and pulmonary edema. According clinical data of study group: we found that; the mean age of all patients was (28.2 ± 5.8) years, the mean BMI was (26.8 ± 4.2), the mean SBP was (169.4 ± 9.3) mmHg, and the mean DBP was (101.6 ± 7.9) mmHg. According Obstetric data of study group: we found that; the median value of previous C/S operations was (1), the median values of GA and GA of delivery were (32) weeks, the median value of gravidity was (3), and the median value of parity was (1). Maternal outcomes, (4%) had DIC, (20%) had HTN, (2%) had HELLP syndrome, (4%) suffered mortality, (4%) had pre and post-eclampsia fits, (2%) had post-partum hemorrhage, and (2%) had renal impairment. Pregnancy (fetal) outcomes, (16%) had IUGR, (38%) had NICU admission, (8%) had pre-term labor, and (2%) had RDS, (12%) had IUFD.
