Abstract Vitiligo is an idiopathic depigmentary skin disorder characterized by selective destruction of melanocytes. Recent observations support the role of altered cellular immunity, autoimmunity, and the role for cytokines in the pathogenesis of vitiligo. The characteristic lesion in vitiligo is a sharply demarcated milky white macule or patch which is surrounded by normal skin. Vitiligo may be divided into localized, generalized and universal. RANTES (Regulated on activation, normal T cell expressed and secreted) is a chemokine responsible for regulating the recruitment of leucocytes into inflamed lesions. It is chemotactic for eosinophils, monocytes, mast cells, basophiles and memory T cells. In vitiligo, RANTES is suggested to play an important role in the pathogenesis of vitiligo via chemo-attracting Th1 cells. Ablative treatment options as fractional ablative CO2 laser, were introduced as a treatment for vitiligo. After fractional CO2 laser therapy various kinds of cytokines secreted during the wound healing process. Also fractional CO2 laser induce activation and proliferation and migration of melanocytes from the outer root sheath into the depigmented epidermis. In the setting of wound healing, it was found that fractional carbon dioxide (CO2) laser enhance fibroblast functions and suppress pro-inflammatory cytokines as IL-1. In the present study, we examined the degree of expression of RANTES in serum of 30 stable non segmental vitiligo patients and evaluate their VASI score (vitiligo area scoring index), and treated with three fractional carbon dioxide laser sessions and daily sun exposure except for the first five days after each laser session, with one month apart between each session, and compared the degree of expression of RANTES with that of 15 normal healthy controls serum, and compare serum RANTES level and VASI score before and after treatment using ELISA kit, serum RANTES was significantly higher in patients than that in controls, and serum RANTES level and VASI score significantly decreased after treatment, so further studies are needed to evaluate the level of RANTES chemokine and other chemokines both locally and in serum. |
