Abstract

SUMMARY
Primary angle closure glaucoma may be preventable in the early stages if we can identify which kind of angles and/or persons are more likely to benefit from prophylactic treatment (lens extraction surgery ) as cortical cataract affect anterior chamber angle width and other parameters measured more than nuclear cataract .
The current trend of diagnosis and management of PACG includes the characteristic optic disc changes and the characteristic gonioscopy findings. The disadvantages of gonioscopy examination include being subjective, time consuming and non-user-friendly. Anterior chamber angle assessment using ultrasound biomicroscopy, an objective method, gives quantitative measurements and can illustrate the mechanism of angle closure but not without disadvantages and limitations. In order to overcome these problems either scheimuflug camera or anterior segment optical coherence tomography can be used being non -contact, objective, rapid image acquisition time and user friendly.
Our study is a cross-sectional study aiming to study the agreement between gonioscopic findings as the standard technique and AS-OCT findings as an objective method for anterior chamber angle estimation in eyes of normal eyes, cortical cataract and nuclear cataract. The study involved 60 eyes (20 normal eyes as control group ,20 eyes with cortical cataract and 20 eyes with nuclear cataract). All the study participants underwent full ophthalmological assessment with gonioscopy using the Goldmann 3 mirror goniolens.
Anterior chamber angle parameters were measured using swept source (opitvue). In our study groups, ASOCT parameters mean and standard deviation showed significant difference in between study groups. Angle width, angle opening distance at 500 & 750 µm in front of scleral spur, TISA 500,750 showed significant difference in between study groups at both nasal and temporal angle imagesOur study showed that there was significant correlation between gonioscopy and most of the studied parameters of AS-OCT. Significant positive correlation was found between the gonioscopy Shaffer classified groups and angle width, angle opening distance, at 500 µm and 750 µm in front of scleral spur ,TISA 500,750.
