Abstract Aim: To compare the effect of a natural herbal licorice containing preventive measure alternative of chlorohexidine preventive measure to decrease the level of salivary Streptococcus mutans in high caries risk patients. Methodology: A total number of 52 high caries risk patients wereassigned in this study. Patients were randomly divided into two groups (n=26) according to type of mouth wash (L) where groups (L1) patients used licorice extract containing mouthwash while groups (L2) the patients used the chlorohexidine containing mouthwash. Then each group was further subdivided into three subgroups according to time (T) where (T0) represented the base line (T1) represented immediately after mouth wash, and (T2) represented one week after using the mouthwash. Furthermore pH and plaque index were tested. Data were statistically analyzed using one-way repeated measures ANOVA followed by Bonferroni post hoc test. Results: It was found that in in high caries risk patients there was no significant difference between licorice and chlorohexidine mouthwashes regarding streptococcus bacterial count. Also it was found that licorice mouthwash stimulating salivary flow thus raising salivary pH in high caries risk patients.However, regarding the plaque index chlorohexidine showed better results.
