Emergency laparoscopy can be used to help reach an accurate diagnosis in addition to its great therapeutic value in management of surgical emergencies. Methods This is a case series study involving a hundred and fifty patients in Cairo University Hospitals{u2019}emergency departments, presenting with acute abdomen who underwent laparoscopy to be able to reach a diagnosis for patients that could not be diagnosed with regular investigations or as a therapeutic tool in acute abdomen conditions diagnosed by other modalities. Results Laparoscopy has a sensitivity of 98.29%, specificity of 100%, positive predictive value of 100%, negative predictive value of 94.29% and accuracy of 98.67% as a diagnostic tool in acute surgical emergencies. As a therapeutic tool laparoscopy successfully treated 88% and only 12% of cases needed laparotomy. Only 4% of cases developed post-operative complications and the average hospital stay for patients was 1.7 days
