Abstract Purpose: The aim of this study is to determine the role of preoperative CT scans on the operative planning and clinical outcome in malleolar ankle fractures. Materials and methods : this prospective study was carried out to determine the effect of preoperative CT scans on the operative planning and clinical outcome in malleolar ankle fractures. Our study was carried out on 84 patients with malleolar ankle fractures. Including; Adult patients with malleolar ankle fractures during the study period diagnosed by x ray, CT scan and operated on. Excluding patients with: Age <18 years, Pathological fractures, Pilon Fractures, history of previous ipsilateral ankle fracture, open ankle fractures, and pregnant or lactating females. Each participant was subjected to: I. Preoperative evaluation :1) History taking, 2) Clinical examination, 3) X-ray imaging of the ankle, 4) CT imaging of the ankle, 5) Laboratory investigations, 6) Comparing X ray and CT findings. II. Operative treatment of malleolar ankle fractures: 1) Preoperative planning, 2) post-operative care. III. Follow up. Results: There was statistically significant difference (p ˂ 0.001) in the diagnosis according to X-ray and CT of the studied patients, and the diagnosis changed in (19%) of patients. Operative decision was changed in 6 patients (7.1%) after CT scanning. There was statistically significant difference between all the suggested decisions depending on X-ray and CT imaging (p ˂ 0.001). Conclusions : Our study affirms that CT scans are more reliable for developing operative plans. As, CT is a useful tool in providing superior detail of fracture patterns and joint congruity and thus it is useful in planning for operative fixation in these fractures. |
