Abstract

The aim of this study was to assess the performance of serum YKL-40 (Human Chitinase 3-like 1) as a diagnostic Biomarker and its discriminating power between different liver diseases. And assessment of the diagnostic value of serum YKL-40 (direct marker) independently and in combination with some of the well-known (non-invasive) indirect markers by our new combination score ( KLT score ) , and study if this combination leading to diagnostic power improvement and their AUCs elevation or not.to fulfill the target of this study , a total of 88 serum samples were collected from El kasr el Ainy ,Abdulkadir Fahmy and el Andalusia Hospital in Egypt ,the age ranges between 40 to 65 years old they classified into 4 groups : group 1 included 20 patients with (HCC) , group 2 including 28 patients with( cirrhosis) , group 3 included 20 patients with (HBV ) , group 4 included 20 patients with (HCV ). The results of the patients were compared to the results of 20 age and sex matched subjects who served as control group. Our study revealed that to differentiate between ( cirrhosis and HCC)& diagnose HBV (control and HBV ) our new established ( KLT score ) is the best for this discrimination as we found that KLT score had the best AUC in comparison to dependent serum YKL-40 ,using KLT score showed improved sensitivity , specificity ,PPV than dependent serum YKL-40 . And we found that for discrimination between (HCV and HBV) & (HBV and Cirrhosis) dependent serum YKL-40 had the best AUC followed by KLT score
