Abstract

Parasitic infestation from swimming in fresh canal water has been well-recognized as an important cause of infectious uveitis in patients from developing countries. Presumed trematode-induced uveitis usually develops in adolescent children with different presentations.
Corticosteroids were the main line of treatment for childhood granulomatous anterior uveitis either topical or subtenon.
The study is prospective comparative study aimed to evaluate the efficacy of subtenon steroid injection alone or in combination with topical steroid in the management of trematode-induced anterior granulomatous uveitis.
The study involved (30 eyes) of 30 cases ”divided equally into tow groups” group A, consisting of patients who received subtenon steroid injection alone and group B, consisting of patients who received a combination of subtenon steroid injection and topical steroid. and the aim of this study was to assess the improvement in their condition by assessing visual acuity, AC activity(cells and flare), granuloma size and ocular complications noted at the time of presentation.
Regarding Sex, the 2 groups had the same sex distribution (p= 1). Age in Subtenon steroid alone group ranged from 7 to 15 with mean±SD = 10.6±2.67 while in Subtnon and topical steroid group the Age ranged from 7 to 13 with mean±SD = 9.6±1.76 with no statistical significant difference (p= 0.238) between the two groups.
Regarding Residence, the two studied groups had the same residence distribution (p= 1). Number of patients with Rural Residence In Subtenon steroid alone group and Subtnon and topical steroid group was 13 (87 %).
Number of patients with Urban Residence in Subtenon steroid alone group and Subtnon and topical steroid group were 2 (13 %).
Regarding Anterior chamber flare, there wasn’t a significant difference between the two studied groups (p= 0.439). Number of patients with anterior chamber flare In Subtenon steroid alone group was 9 (60 %), while in Subtnon and topical steroid group number of patients was 11 (73%).
Follow up IOP in Subtenon steroid alone group ranged from 10 to 16 with mean±SD = 12.4±1.64 while in Subtnon and topical steroid group the Follow up IOP ranged from 10 to 16 with mean±SD = 12.67±1.88 with no statistical significant difference (p= 0.682) between the two groups.
Regarding Follow up Visual acuity, There was a highly significant difference between the two studied groups (p= <0.001). Number of patients with 6/6 Follow up Visual acuity In Subtenon steroid alone group was 0 (0 %), while in Subtnon and topical steroid group number of patients was 9 (60 %). Number of patients with 6/9 Follow up Visual acuity In Subtenon steroid alone group was 2 (13 %), while in Subtnon and topical steroid group number of patients was 4 (27 %). Number of patients with 6/12 Follow up Visual acuity In Subtenon steroid alone group was 7 (47 %), while in Subtnon and topical steroid group number of patients was 2 (13 %). Number of patients with 6/24 Follow up Visual acuity In Subtenon steroid alone group was 6 (40 %), while in Subtnon and topical steroid group number of patients was 0 (0 %).
Number of patients with (2) SUN Grade In Subtenon steroid alone group was 6 (40 %), while in Subtnon and topical steroid group number of patients was 2 (13 %). Number of patients with (3) SUN Grade In Subtenon steroid alone group was 7 (47 %), while in Subtnon and topical steroid group number of patients was 0 (0 %). Number of patients with Persistent granuloma In Subtenon steroid alone group was 15 (100 %), while in Subtnon and topical steroid group number of patients was 0 (0 %). Number of patients with Healed granuloma In Subtenon steroid alone group was 0 (0%), while in Subtnon and topical steroid group number of patients was 13 (87%). Number of patients with Disappeared Granuloma In Subtenon steroid alone group was 0 (0 %), while in Subtnon and topical steroid group number of patients was 2 (13 %).
