Abstract compare the maternal outcomes of women who underwent Hysterectomy vs those who underwent Conservative Surgery for placenta Accreta spectrum. Materials and methods: This study was carried out on 100 women with a diagnosis of placenta accrete spectrum who had conservative surgery or hysterectomy at Mansoura University Hospitals were the subjects of a clinical controlled non randomized interventional study. All participants were recruited - during the period from 21 June 2021 to 30 December 2022 in obstetrics and gynecology, Mansoura University Hospitals, Mansoura, Egypt. Results: •There is no statistically significant difference among both groups as regard age, BMI, Residency, previous obstetric and gynecological history. •There is statistically significant difference among both groups as regard Gestational age, GA in Second CS, Operative time, need for blood products transfusion, Hemoglobin assessment after surgery, Blood loss (ml), Hospital stay, Postoperative complications. Conclusions : Conservative placenta accreta management reduces maternal morbidity and mortality compared to surgical hysterectomy management in well selected patients who have desire to retain their fertility.
