Abstract PPHN results when the normal circulatory transition fails to occur at birth, resulting in elevated PVR. PPHN carries mortality rates of up to 10% and is associated with long term morbidities including high rates of neurodevelopmental impairment. HFOV has been used with increasing frequency for PPHN, and when combined with inhaled NO results in an acute rise in arterial oxygen saturation and reduces the likelihood of ECMO (treatment. Likewise, HFOV improves oxygenation at lower mean airway pressures and ventilation of infants with PPHN, as compared to conventional ventilation. Although HFOV is used frequently in NICU, there are few systematic studies of its hemodynamic effects with contradictory results. The present work is a prospective cohort study which aimed to evaluate the hemodynamic status and cardiac performance in neonates with PPH on HFOV and CMV before and after treatment by EC in comparison to echocardiography. |
