Abstract

Background: All societies are affected by traumatic brain injury (TBI) which is considered a global public health crisis, economic, and social challenge. Many nations consider it the leading cause of death and disability. TBI management has changed dramatically due to advances in critically ill patient care & a better understanding of secondary brain injury. Cognitive problems often follow severe TBI. Agitation is common among coma survivors. Aim and objectives: The aim of the research was to assess the effectiveness & safety of atypical antipsychotics (Risperidone & Quetiapine) in the management of agitated behaviors following TBI.
Subjects and methods: This prospective randomized controlled study was conducted in trauma ICU of Minia University Hospital on 60 adult patients aged 18-60 years old with isolated moderate TBI, starting from date July 15, 2022 to May 30, 2023
Result: there was a significant difference between the three studied groups regarding GCS 7th day. GCS in the 7th day measurements were significantly lower among group R compared to group Q. RASS at day was significantly lower in group R compared to group Q than group C from 3rd day to 9th day. Also, RAAS at night was significantly lower in group R compared to group Q than group C from 2nd day to 9th day. ICU length of stay was significantly lower in group R compared to group Q and C
Conclusion: The study found that Risperidone & Quetiapine were effective & safe for TBI-related agitation. This study found that Risperidone is better than Quetiapine for TBI-related agitation. Risperidone improved RASS & reduces ICU stay more than Quetiapine. However, Risperidone reduced MAP and increased HR and RR without clinical relevance.
