Abstract

Tonsillectomy is a relatively simple but major surgery because anesthesia and postoperative complications including pain and hemorrhage. Bipolar diathermy and suture ligation are used methods for controlling pain and hemorrhage during and after tonsillectomy. While many studies demonstrated the outcomes of suture ligation and bipolar diathermy methods, no available studies compared the outcomes of bipolar diathermy with and without ligation of the lower pole. Therefore, the rationale of this study is to compare the postoperative pain and bleeding after bipolar tonsillectomy with and without ligation of the lower tonsillar pole.
Hence, this study aimed to compare the outcomes of bipolar tonsillectomy with and without ligation of the lower tonsillar pole.
A Randomized controlled study was conducted in the Department of ENT, Suez Canal University Hospital. Forty patients were enrolled. Informed consent and demographic information were taken from all the patients. Tonsillectomy was done with bipolar diatheramy. In twenty patients (group A), the lower pole was cauterized for hemostasis and in the other 20 patients (group B) ligation technique was used. The postoperative pain and bleeding in both groups were compared. The collected data was entered and analyzed on SPSS version 22.
A total of 40 patients (65% female) underwent tonsillectomy, mostly due to recurrent tonsillitis and mainly grade 3 tonsil enlargements. Bipolar tonsillectomy without Ligation of the Lower Tonsillar Pole was superior to the ligature approach in terms of duration of operation, intraoperative bleeding and need to intraoperative stitches, less post-operative pain scores, faster healing and more rapid recovery.
