Abstract Background: The pathogenesis of seborrheic dermatitis has not been fully elucidated. It is considered to be a multifactorial disease. The complex interplay of Malassezia, keratinocytes and the immune response against an altered lipid composition in the skin plays a crucial role in its pathogenesis. A large number of inflammatory markers have been noted to be increased in SD. Interleukin 22 is a member of IL 10 family which mediates its effects via the IL--22 receptor complex. IL--22 can induce the proliferation of human epidermal keratinocytes obtained from healthy individuals suggesting that it may have a role inducing epidermal hyperplasia encountered in SD. Aim and objectives: The aim of this study was to highlight the role of IL-- 22 in seborrheic dermatitis pathogenesis to improve the management of this common skin disease, Subjects and methods: This case control study was conducted at Dermatology Outpatient Clinic, Suez Canal University Hospitals, Ismailia and included 50 patients with seborrheic dermatitis and 50 healthy age and gender matched controls without history of seborrheic dermatitis, systemic infections, personal or family history of atopy or autoimmune disease. The serum level of IL--22 was measured by enzyme linked immunosorbent assay in patients with SD and healthy controls. Results: The present study showed that mean ± SD of IL––22 in patients with SD was 56.56 ± 7.8 pg./ml, and in control group was 8.60 ± 1.68 pg./ml. Serum IL--22 level was significantly higher among patients with SD than in healthy controls(p<0.001) .IL--22 level was statistically significant higher among patients with advanced SEDASI score than in patients with mild and moderate SD (p=0.001). Conclusion: IL--22 serum level was higher among patients with SD which may highlight the role of IL--22 in the inflammatory reactions, epidermal proliferation and epidermal barrier dysfunction encountered in SD.
