Abstract The epithelial d group antigens are glycoprotein or glyco-lipid complexes. Their antigenicity is derived from their oly-saccharide side chain which is synthesised as a wise elongation under the control of glycosyl ferase enzymes. They are present in all body tissues the Central Nervous System 78% of the human show blood group antigen activity in all their s cretions and body fluids except the Cerebro Spinal Fluid nd they are known as secretors. The blood group are lost early during the process of malign transformation in different organs. This study of epithelial blood group antigens in different laryngeal lesions included 50 cases of differ~nt pathological lesions and. it proved that, the blood group antig ns are not affected in benign lesions and low grade dysplasia of the larynx. But, they are affected in potentially malignant lesions e.g recurrent papilloma and high grade dysplasia. This study proved the exist~ ce of a direct relation between the behaviour of the malignant tumour and its grade on one hand and the loss of the isoantigens on the other hand. Thus, the epithelial blood group antigens may help to detect ea ly malignant change in benign lesions while they are still at a high curable rate.
